The Challenge

Contact Health has over 20 years of experience in providing supportive healthcare and health monitoring solutions.

They had a vision for a dedicated, web-based remote patient monitoring and care management system that would provide a safety net for at-risk individuals living at home.

The software solution needed to embrace Internet of Things (IoT) technology and advanced wireless and wearable tech devices. The purpose was to provide a gateway for communication between patients and the health practitioners, in ways never imagined before. The system would help caregivers manage health risks, improve the outcomes of health procedures, transform the delivery of healthcare products, reduce readmissions, safeguard lives, and optimize healthcare procedures.

The client’s goal was a system that could assess the patient's journey from one point in time to another. In this way, the system had to create a holistic timeline with all the vital statistics, such as blood pressure, pulse, weight, glucose, and temperature, over an observable period. The system would then map these values against a normal-value chart. If the patient data showed a variation from normal, the system would alert the remote care staff, who could then take remedial action to address the patient’s health needs.

Hence, Taazaa had to feed complex rules for alert generation into the system, with the option to customize them.

The Solution

The system reads patients’ health parameters such as body temperature, heart rate, glucose, and others via advanced wireless and wearable tech devices supplied by the client.

Then, the system sends these parameters to medical service providers. Upon the receipt of patients’ data, medical service providers perform their analysis and suggest an appropriate care plan. Care plans include medication prescriptions, a periodic questionnaire containing health queries, and educational resources for the patient.

Taazaa developed a custom healthcare system using robust web and mobile technologies, IoT integrations, forms, surveys, and HIPAA-compliant messaging to deliver remote monitoring and a patient experience tailored to each patient's needs.

The patient module is implemented using native iOS and Android development technologies. These patient modules are pre-loaded onto the client-provided mobile devices for patients.

The patient module includes many features, such as alert generation for medical practitioners, report generation, medicine reminders, medical alerts, vital signs monitoring, medication management, program services, and medical monitoring.

The messaging between the patient module and the Contact Health system is two-way, and the doctor can also pass messages to patients if he sees any discrepancies in health parameters. It also sends messages when any parameter crosses its critical value.

The system works over phone lines, broadband, VoIP, and cellular networks. The application connects users and trained response teams instantly based on configured alert systems.

The Results

The software developed is pre-loaded on a tablet and includes video education, conferencing, and health assessments. Users use the tablet to fulfil treatment plans and remain engaged in self-care activities.

In case of help, patients access tablet to immediately access caregiver, clinician, or tech support. The application works on data input to track activity, diet, vital signs, weight, medication available, and can be reviewed by clinicians or caregivers at any point in time.

Caregivers and medical practitioners receive alerts to intervene if the patient is at increased risk for urgent care or hospital readmission.

The application has already served more than 3,500 patients, with adherence rates between 74.5% and 77%, resulting in improved clinical outcomes.
