Our client is an expert group of health safety executives with an experience of over 20 years in provisioning health supportive care and health monitoring solutions.
The client wanted a dedicated web-based Remote Patient Monitoring and Care Management System to be able to provide a safety net for individuals while they live at their homes with comfort.
The software solution to be developed should be such that it takes up maximum utilization of advanced wireless and wearable tech devices. The purpose was to provide a gateway for communication between patients and the health-practitioners, in ways never imagined before. Beyond that, the purpose of the solution also aimed at managing health risks, improving outcomes of health procedures, transforming the delivery of healthcare products, reducing readmissions, safeguarding lives, and optimizing health care procedures.
The client’s needs could be fulfilled by a system that could assess the journey of the patient from one point of time to another. In this way, the system had to create a holistic timeline with all the vital statistics such as blood pressure, pulse, weight, glucose, and temperature, over an observable period. The system would then map these values against a normal-value chart. Generation of alerts would happen in the case of any of these going out-of-bound. As such, medical practitioners sitting at a remote location could be able to prescribe medication or specific other remedial advices.
Hence, Taazaa had to fed complex rules of alert generation in the system with the additional option of defining these rules on a custom basis.
Remote Patient Monitoring and Care
Management have evolved with the
advent of new age technologies.
The system is used to read patients’ health parameters such as body temperature, heart rate, glucose, and others via advanced wireless and wearable tech devices supplied by the client.
Then Remote Patient Monitoring and Care Management System sends these parameters to medical service providers. Upon the receipt of patients’ data, medical service providers perform their analysis and suggest an appropriate care plan. Care plans include medicinal prescription, a periodic questionnaire containing health queries, and other engaging resources for the patient.

Patient-module has many features such as alert generation for the medical practitioner, report generation, medicine reminder, medical alerts, vital signs monitoring, medication management, program service, and medical monitoring.
The messaging between the patient-module and Remote Patient Monitoring and Care Management System is two-way, and the doctor, too, could pass on messages to the patients in case he sees any discrepancy in the health parameters. It also sends the messages when any parameter crosses its critical value.
Our system works on phone lines, broadband, VoIP technologies, and cellular networks. The application connects users and trained response teams instantly based on various alert systems configured.
The software developed is pre-loaded on a 4G tablet and includes video education, conferencing, and health assessments. Users use the tablet to fulfil treatment plans and remain engaged in self-care activities.
In case of help, patients access tablet to immediately access caregiver, clinician, or tech support. The application works on data input to track activity, diet, vital signs, weight, medication available, and can be reviewed by clinicians or caregivers at any point in time.
Caregivers and medical practitioners receive alerts to intervene if the patient is at increased risk for urgent care or hospital readmission. The application has already served more than 3500 patients with patient adherence rates between 74.5% – 77%, resulting in improved clinical outcomes.
